Patient Referral

• Referrals can be sent via Medical Objects, faxed to (07) 3112 6885 or via email – Brisbane: [email protected]  or Sunshine Coast: [email protected]
• WorkCover and CTP referrals are accepted for treatment only (not Independent Medical
  Examinations IME)

Information needed to expedite the progress of a referral:

• A current medication list or a pharmacy review prior to appointment
• An outline of relevant past medical history
• Copies of reports (X-rays, MRIs etc.) relating to the condition
